What to include
Chain identification
Existing chain number, measured pitch, strand count, roller diameter, inner width and any visible markings.
Drive geometry
Sprocket tooth counts, shaft centres, bore and hub details, plus photos showing alignment and guarding.
Operating duty
Machine function, speed, load, starts and stops, shock, daily operating pattern and any known failure mode.
Environment
Dust, particle type, moisture, wash-down, temperature, cleaning practice and current lubrication method.

Before sending
I only know the pitch. Is that enough?
Pitch narrows the search, but inner width, roller diameter, pin diameter, plate geometry and strand count should also be confirmed.
Should I include the sprockets?
Yes. Tooth count and tooth condition are important, particularly when a chain has elongated or the existing teeth are hooked.
